Breast localization codes now require device HCPCS codes when billing under OPPS

Q.

Our hospital is receiving an edit for CPT® code 19285 stating that we need a device code. We use needles for breast localization. Is there an appropriate HCPCS code that I should be adding to the claim, to decrease or stop this denial?

A.

This is a problem when you aren’t using a clip or other totally implantable device. Some Level II HCPCS code suggestions might be A4649, A4212, or A4215. For totally implanted devices, you would use A4648.

The codes and full descriptions are as follows:

A4212 Noncoring needle or stylet with or without catheter

A4215 Needle, sterile, any size, each

A4648 Tissue marker, implantable, any type, each

A4649 Surgical supply; miscellaneous
