Documenting concurrent supervision for CD volume-rendered images

 What is “concurrent supervision” in relation to the 3D CPT^® codes 76376 and 76377, and how must it be documented? 

A.

Concurrent supervision defines a “temporal relationship” to creating the 3D volume-rendered images. The physician must actively participate in and monitor the reconstruction process, including: 

• Design of the anatomic region to be reconstructed;
• Determine the tissue types and structures to be displayed (e.g., bones, organs, vessels);
• Determine the images or cine loops to be archived; and 
• Monitor and adjust the 3D work product. 

If the physician is providing supervision as above, a note in the dictated report that “the 3D reconstructions were performed under my concurrent supervision” would suffice for documentation.
